Instructions for Downloading Standard Initial Practice Forms

Our standard practice forms for new patients are now available here for downloading and your conveni8ence. 

New patients can: (1) DOWNLOAD these forms (Initial Patient Face Sheet & Consent Forms, Case History, Release of Information, Verification of Receipt of HIPPA Regulations, HIPPA Notification of Privacy Practices); (2) PRINT them out in color; and (3) COMPLETE them at your own convenience. Thus, on the day of your first appointment you will be well-prepared and only need to be at our office just 10-15 minutes before your first appointment. .

Forms are all in Microsoft Word or Microsoft Publisher format; but, all our brochures are in Microsoft Publisher format. If you do not have either Microsoft Word or Microsoft Publisher, let us know and we can send you the PDF version. Another option is tyo go directly to www.openoffice.org and download their free software package. Open Office programs are all compatible with Microsoft Word and Microsoft Publisher documents. We look forward to speaking with you soon.
